Including writing reports made to contracted facility and patients’ physician. Plan, organize and conduct therapy programs in hospital, institutional or community settings to help rehabilitate those impaired because of illness, injury, and psychological or developmental problems. Assists the patient, family, and appropriate restorative personnel to obtain information relative to the specific disorder or disorders while in rehabilitation setting to facilitate patient recovery. Interacts regularly with other members of the rehabilitation team and nursing staff on the patients treatment plan, progress and prognosis. Evaluates patients progress and prepares reports that detail progress. Assures that all required written documentation is completed and appropriately placed on a timely manner. Tests and evaluates patients physical and mental abilities and analyzes medical data to determine realistic rehabilitation goals for patientsOrganizational Relationships:Supervises:Responsible for the care delivered by COTAs.Essential Job Responsibilities:- Provides differential diagnosis for patients with functional disorders.
